1 DAY COURSE

Introduction

The Incident Investigation course is based on two formal guidance documents:

  1. British Standard BS 45002-3:2028 Occupational H&S management systems - General guidelines for the application of ISO45001, Part 3: Guidance on incident investigation

  2. The well renowned, UK HSE HSG245 Investigating Accidents and Incidents Guidance

The Incident Investigation course is delivered over by a 1 day tutor-led remote training session via Zoom by Andrew Protheroe (MSc, CMIOSH, FIIRSM, FInstLM, CQP MCQI)

 

Course Objectives

The main objective of the Incident Investigation course is to ensure organisations and are properly prepared to manage, evaluate and improve their response to incidents.

Course Content

1 - Pre Incident Phase

  • Emergency preparedness (communication, competence, scenario planning)

  • First Aid Needs Assessment

  • Incident procedures

  • Reasons for investigations

  • Performance Influencing Factors (PIFs) that can contribute to incidents

  • Cognitive bias in investigations

2 - Incident Phase

  • Incident response

  • The process for investigating incidents

  • Types of evidence (Physical, Verbal, Written)

  • Interview techniques and barriers to effective interviews

  • Document review

3 - Post Incident Phase

  • Non-conformity recording and reporting

  • Improvement - Remedial action plans

Assessment

  1. Pre-course 30 Question Self-assessment

  2. Post-course 30 Question Assessment, 28 to Pass over 30 minutes

Certification

PCR Global Certification (no expiry).
